45 / hourThe Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention. The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span. Essential Duties and Responsibilities:

Provides necessary social work intervention

Counsels individuals and families in crisis and help with issues such as poverty, loss and illness

Assesses patient's psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient/client goals

Collaborates with all disciplines to plan and evaluate team goals for each patient/client

Facilitates patient/client and family referrals to community agencies when appropriate

Provides financial counseling when necessary to achieve the treatment goals established by the multi- disciplinary team

May be required to respond to crisis situations, including domestic violence and child abuse

May be required to provide witness services and evidence in court cases

Visits schools to assess academic concerns

Works with government agencies to help clients apply for Medicare and Medicaid when appropriate

Arranges home visits to evaluate a patient/client health, home environment and family interaction/support

Assists patient/family/caregivers in planning for present and future psychosocial needs

Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members

Adheres to facility and Amergis policies and procedures at all times

Minimum Requirements:

Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required

Active license as an LCSW in state of practice required

Minimum of one year of social work experience preferred

Complies with all relevant professional standards of practice

Participation and completion of Amergis' Competency program when applicable

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable to job site

Understand patient confidentiality and HIPAA requirements

Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

Computer proficiency required

Must be at least 18 years of age
