Program Description:

Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable experiencing homelessness. The TLC Rapid Re-Housing (RRH) Program provides housing and comprehensive supportive mobile case management for youth, 18-25, experiencing homelessness. Referrals are accessed through Sonoma County’s Coordinated Entry Database (CE) and are enrolled in our program through the Homeless Management Information System (HMIS). TLC partners with landlords and homeowners throughout Sonoma County to provide safe housing for our youth to live, while operating within a Housing First paradigm. Services are focused on assessing and addressing a young adult’s specific barriers to accessing housing (i.e. substance use, mental health symptoms, criminal background, finances) with the end goal of supporting them in obtaining long term affordable stable housing. Participants receive support from our trauma-informed dedicated staff within all areas of independent living including employment, education, finance management, accessing resources, and navigating the ins-and-outs of learning to live independently.

Position Summary:

TLC Child and Family Services is seeking a full-time (40 hrs/week) Case Manager for our Rapid Rehousing Program. Under the supervision of the Program Director, and reportable to the Chief Executive Officer of TLC, it is the responsibility of the Case Manager to:

Contact potential participants from the CE By Names Waitlist to determine their interest in our services.Conduct intake interviews to identify youth’s housing needs, and interest in the program, while adhering to Housing First principles.Coordinate with the Housing Director, Housing Navigator, and Program Director to link youth housing needs to TLC’s available housing.Complete all mandatory HMIS documentation for youth in the program, including an initial intake, a yearly update (as needed), quarterly reports, and an exit assessment.Complete monthly case notes.Attend training relevant to homeless system services and or offered by agencies providing our funding for this program.Represent TLC, specifically related to this program, in community meetings or collaborative efforts.Knowledge of Rapid Rehousing and Various Voucher eligibility and guidelines. Case Managers have a caseload of no more than 15 and are expected to engage in weekly contact and support, including at least once a month in the home. This position requires teamwork, dedication, commitment and patience in developing effective treatment plans for program participants, while implementing state, county and agency policies and procedures. This position requires flexibility with personal scheduling and meeting a variety of needs as assigned on a daily basis by the immediate supervisor. Some on call, night and weekend hours needed.Essential Functions:Conduct assessments of client strengths and needs and advocate for them accordinglyAid each participant as they develop and meet the goals established in their Initial Assessment, outlining personal, financial, educational, employment and housing objectivesMaintain case records and provide timely reports on an individual participant’s progress and outcomesFacilitate admissions through the CE By Names Waitlist and work collaboratively with the Housing Director and Program Director to identify available housing options

Wage range: $23-$25.89/hour, depending on education and experience. Full-time positions include health benefits, paid time off, and a discretionary retirement program.
