Care Partner
About the role:
In this role, you will manage a caseload of community-based elderly individuals with Home Care Packages in Blacktown & surrounding areas. You will assess their needs, develop personalised care plans and manage budgets in accordance with program guidelines, to ensure each customer's unique requirements are met. You'll also onboard new customers and proactively manage existing ones with a person-centred approach. Collaboration with our Clinical, Scheduling and Service Delivery teams will be crucial to delivering comprehensive care.
This is a 12-month fixed term contract, and you will have the flexibility to manage your own schedule each week, while working autonomously from home, our Bella Vista office and within the community. As you will regularly visit customers in their homes, you will need to be based close to, and willing to travel, within this region.
About you:To be successful in this role, you will have:Ideally proven experience in community-based case managementExperience with managing Home Care Packages and budgets is advantageousCurrent driver’s licence and a reliable vehicle with minimum Third-Party insuranceStrong communication skills, both verbal and writtenDemonstrated empathy and compassionExcellent problem-solving abilities and critical thinking skillsGood organisational and time management skillsCultural competence and the ability to work with diverse populationsAdvocacy skills to ensure clients' needs are metCollaboration and teamwork skills to work effectively with interdisciplinary teams
